The NHS Friends and Family Test

Access to your medical records. You can now apply to access your medical records online. To do this you will first need to be registered for Patient Online Services. Apply at reception by completing a request form.

AccuRx is replacing eConsult from 25/03/2024. You will be able to submit any online request through AccuRx – click on the banner below or via NHS app or via NHS website.

AccuRx is the new way to contact your practice online which helps you access the help and advice you need at the right time, in the right place, with the right person.

HOW ARE PRACTICES WORKING NOW?

BOOK YOUR APPOINTMENTS ON LINE there will be a limited daily number to allow for patients to book via telephone or at the Practice.

So that the people with greatest need are seen first; and so we don't ask people to travel and come into contact with unwell and infectious people, you will be assessed to decide who needs:

  • to be seen in person by one of the team
  • a phone consultation
  • a video consultation
  • help from the community pharmacy or another health service

WHY DO RECEPTIONISTS ASK PERSONAL QUESTIONS?

GP reception staff are skilled professionals and ask questions to make sure you see the right person at the right time, and treat all information confidentially. If you don't want to speak on the phone, where possible fill in an online form on the practice website, this is known as AccuRX.

I WANT TO SEE MY GP, SO WHY AM I SEEING SOMEONE ELSE?

Many GP practices now include a range of professionals like nurses and paramedics. This means you can often be seen more quickly and not need to be seen by lots of different people.

EMAILS FROM PATIENTS

Please be aware that messages sent to our email address are read at 09:30, 12:00 and 16:00 Monday to Friday, but only requests vetted and deemed urgent by the Practice will be extracted and processed after 16:00

REPEAT PRESCRIPTIONS

You can now request repeat prescriptions and view your medical record online by registering with the NHS App or Online Patient Services. Please also visit the surgery website: www.drdaviesandpartner.nhs.uk for AccuRX.

DO NOT USE EMAIL FOR SEEKING URGENT MEDICAL ADVICE FROM A HEALTHCARE PROFESSIONAL.

Please do not request home visits or appointments via email. You will need to call the Surgery on 01322 449327. Requests of this nature received via email will not be responded to. For home visits please call the surgery on 013222 449327 and press option 3 before 11:00

Prescription requests take a minimum of 72 working hours to be processed. Some requests will need to be reviewed by a GP before they can be issued and may take longer to process. We will endeavour to deal with any urgent prescription requests on the same day. Unfortunately, we cannot email patients to confirm that their prescription has been processed.

Fit Notes (Sick Certificates) take 48 hours to be processed. Please request these via AccuRX. Employees only need a sick certificate from a Doctor after being off sick for more than 7 days. It is therefore Practice policy not to provide patients with a certificate for any period of less than 8 days. We are  also unable to issue an extension before the end date current certificate. There is no longer any requirement for doctors to provide notes stating that their patient is fully fit for work. It is up to the employer to decide whether you are fit to carry out your role.

From 1 July 2022, legislative change has enabled a wider range of healthcare professionals to certify fit notes. The healthcare professionals who can now certify fit notes in addition to doctors are nurses, occupational therapists, pharmacists and physiotherapists.


All GP practices are required to declare the mean earnings (e.g. average pay) for GPs working to deliver NHS services to patients at each practice.

The average pay for GPs working in Dr Davies & Partner in 2021 was £82,168 before tax and National Insurance.

This is for 3 part time GP's who worked in the practice for more than six months.

Morning, evening, and Saturday appointments

We want to make it easier for you to get an appointment at our surgery. In addition to our normal opening hours, we also offer appointments in the morning, evenings and on Saturdays. You will be offered a face to face appointment, but your appointment may also take place by phone or video. We are working with our neighbouring GP surgeries to offer these additional appointments, you may be offered an appointment from one of these other surgeries. Our surgery is part of a small group of surgeries called a Primary Care Network and we work together in a number of ways. Our neighbouring GP surgeries in the Primary Care Network are:

What is confidential patient information?

Confidential patient information identifies you and says something about your health, care or treatment. You would expect this information to be kept private.

Information that only identifies you, like your name and address, is not considered confidential patient information and may still be used:

For example,

  • To contact you if your GP practice is merging with another.

Who can use your confidential patient information for research and planning?

  • It is used by the NHS, local authorities,
  • University and hospital researchers, medical colleges and pharmaceutical companies researching new treatments.

Making your data opt-out choice

You can choose to opt out of sharing your confidential patient information for research and planning. There may still be times when your confidential patient information is used: for example:

During an epidemic where there might be a risk to you or to other people’s health. You can also still consent to take part in a specific research project.

Will choosing this opt-out affect your care and treatment?

No, your confidential patient information will still be used for your individual care.

Choosing to opt out will not affect your care and treatment. You will still be invited for screening services, such as screenings for bowel cancer.

What should you do next?

You do not need to do anything if you are happy about how your confidential patient information is used.

If you do not want your confidential patient information to be used for research and planning, you can choose to opt out securely online or through a telephone service.

You can now book your doctors appointment on line and order repeat prescriptions visit www.patient-services.co.uk/

You will need to register for this service at reception and obtain a token advising of your first login details. You can also request access to your journal online by application at reception. If you have any questions queries regarding your medical journal and entries within it, you should make an appointment to see or speak with a clinician. Please do not ask reception as they are not qualified to answer such questions.

Get Well, Keep Well

Of course we’re not just here for when you are unwell. Our team of healthcare professionals and back-up staff offer a number of clinics and services to promote good health and wellbeing whatever your medical condition.

Access

A ramp and double doors are provided at the entrance to the surgery for ease of access. If you require any assistance please ask at reception. The patient toilet  allows for a wheelchair and has disabled facilities.
