The NHS Friends and Family Test

Access to your medical records. You can now apply to access your medical records online. To do this you will first need to be registered for Patient Online Services. Apply at reception by completing a request form.

If you are contacting the practice to book an appointment, please complete an online consultation (through AccuRX), you can do this by visiting our website

If you do not have access to the internet or are unable to complete the form on AccuRX, please contact the surgery on: 01322 449 327.

Registration takes place here, at Littleheath Road Surgery, every day that the practice is open.

Firstly, to register as a patient at this surgery, you need to ensure you live within our practice boundary.

If you live within our boundary and would like to continue registration with us, you will need to:

  • Complete a form called a GMS1 Registration Form Pre-registration Form , providing details such as your name, date of birth, NHS number (if you know it), other information such as the name and address of your previous GP and your views on organ donation
  • Provide ID/residence documentation to verify that you live within our practice boundary


Once the GMS1 form is returned to us, we send the details  to PCSE, your registration is then complete and your medical records are transferred to us.

 Health checks

When you register with our surgery, you will be invited to make an appointment for a health check within six months.

Health checks are usually undertaken by our practice nurse, who will ask you about your personal and family medical history. They will also ensure that any tests or checks you need are up-to-date, such as measuring your blood pressure or arranging cervical screening.
