The NHS Friends and Family Test

Access to your medical records. You can now apply to access your medical records online. To do this you will first need to be registered for Patient Online Services. Apply at reception by completing a request form.

If you are contacting the practice to book an appointment, please complete an online consultation (through AccuRX), you can do this by visiting our website

If you do not have access to the internet or are unable to complete the form on AccuRX, please contact the surgery on: 01322 449 327.

Patients are able to book directly into an appointment of their choice via the link or QR code below .  Walk-in appointments are also available (but booking is preferred to allow triage ahead of the vaccination & to prevent patients 
having to wait around )

Catch-up vaccination is available for anyone not fully vaccinated, including children, teens and adults.

IT'S NOT TOO LATE TO CATCH UP.  MANY CHILDREN MISSED VITAL ROUTINE VACCINATIONS OVER THE PAST COUPLE OF YEARS, PUTTING THEM AT RISK OF PREVENTABLE DISEASE.  THE GOOD NEWS IS THAT IT'S NOT TOO LATE TO PROTECT YOUR CHILD FROM SERIOUS ILLNESS.

MAKE AN APPOINTMENT WITH THE PRACTICE NURSE AS SOON AS YOU CAN, OR VISIT:

KEEPING UP TO DATE WITH IMMUNISATION HELPS TO PROTECT YOUR BABY OR CHILD AGAINST 17 DISEASES such as:

Whooping Cough            Measles

Septicaemia                    Tetanus

Diphtheria                       Polio

Meningitis                       Rotavirus

Flu Vaccine is offered each year to:

Children aged two or three years old (on 31 August of current flu season)

and

Children with a health condition that puts them at greater risk from flu.

Whooping Cough (Pertusis) in Pregnancy

Expectant mums can help to protect their babies by being vaccinated from week 28 of their pregnancy
